Family-Managed Home Care program from the HCCSS

The Family-Managed Home Care program provided by the Local Health Integration Network (LHIN)  now called the “Home and Community Care Support Services” (HCCSS) is designed to give families greater control over the care of their loved ones at home. Through the Family-Managed Home Care program, eligible patients or their substitute decision-makers, receive directly funding each month from the Government.

Families can use this fund to hire First Support Health Care. We can provide you our certified and professional Personal Support Worker (PSWs), Registered Practical Nurses (RPN) and Registered Nurses (RN) for home care services tailored to your needs and the HCCSS will give you money to help pay your invoice.

Families work closely with care coordinators from the HCCSS to develop personalized care plans tailored to the specific needs of their loved ones. This ensures that the care provided aligns with the individual's preferences, health conditions, and lifestyle.

The Family-Managed Home Care program offers training and support to families to help them effectively manage their caregiving responsibilities.

While families have greater autonomy in managing care, the HCCSS provides ongoing monitoring and oversight to ensure that the care provided meets quality standards and that the needs of the individual receiving care are being met.

Overall, the Family-Managed Home Care program empowers families to take a more active role in caring for their loved ones at home while still providing access to the necessary support and resources from the HCCSS.
